The Health Insurance Portability and Accountability Act is effective April 14, 2003. This is a federal government regulation that protects your privacy as a patient.  Here at TENT, patient privacy has always been a top priority. 

Because of this new federal regulation you will need to complete some additional paperwork. All patients (even return patients) will need to complete a new demographic profile. 

We are regulated by the US government and have to take these steps to meet the new guidelines.  These guidelines were set for the protection and privacy of your health information.
